We are currently scheduling appointments for fuel and electric assistance for the coming season.  Please note that the fuel program will not officially open until December 1, 2024.

Below is a list of documents that you will need to bring to your appointment

  • Social Security Numbers and Dates of Birth of all household members
  • Verification of All Household Income. - You will be required to show verification of all household income for the 30 days prior to your appointment. Verification of income includes, but is not limited to:
    • Employment Pay Stubs (5 pay stubs if paid weekly; 3 pay stubs if paid bi-weekly; 2 pay stubs if paid monthly)
    • Social Security Award Letters (including SSA, SSI, SSDI and/or Survivor Benefits)
    • Unemployment
    • Pensions
    • Self-Employment (will require signed current tax return with all schedules)
  • Landlord Verification: If your heat is included in your rent your landlord will need to complete a Landlord Verification Form. Landlord Verification Form PY25
  • State and City Assistance: You will need a copy of your most recent decision letter from your local Town Welfare Office, or the Department of Health and Human Services (Food Stamps, TANF, APTD, etc.) please include all pages.
  • Child Support Paid Out or Received: Child Support Verification Form PY25 
  • No/Low Income Form: Must be completed by anyone in the household who is 18 years or older, is not in high school, and has either No or Low Income. No or Low Income Declaration Form PY25 
  • Tax Return: All household members that have filed must provide a copy of their completed IRS tax return. Any household member that is Self-Employed is required to provide a copy of their most recent signed taxes with all schedules. Please note that as of March 1 of each year, your current year taxes are required. For any adult members that did not file, you will be asked to complete a 4506-T form.   • Copy of your heating bill.
  • Copy of your electric bill.

Note: This is a list of common documents needed for the applications for our services. We cannot address each household's individual circumstances, and we may need to request additional information to complete your application.

Failure to bring all necessary documents with you to your appointment will delay your application process.
